Quick Guide: Correct AED Pad Positioning

For adults and children 8 years or older: Use the front-side placement – one pad on the upper right chest just below the collarbone, the other on the lower left chest along the mid-axillary line below the nipple.

For children younger than 8: Use front-back placement – one pad in the center of the chest, the other on the upper back between the shoulder blades.

For infants: Use pediatric pads in front-back position. If pediatric pads are unavailable, adult pads may be used in the same positions.

Understanding Different AED Pad Placement Methods

Anterolateral Placement (Standard for Adults)

Recommended for: Adults and children over 8 years (over 55 lbs)

Right pad: Upper right chest, below the collarbone and next to the sternum

Left pad: Lower left chest, below the nipple along the mid-axillary line

Avoid bones, nipples, and direct placement on the sternum

This path maximizes current flow through the heart for effective defibrillation

What is Anteroposterior Placement?

Anteroposterior placement means one pad on the front of the chest and one on the back. This is the preferred method for infants.

Anterolateral placement places both pads on the front of the chest, on opposite sides.

One pad goes on the upper right chest below the collarbone. The other goes lower on the left chest, below the nipple and toward the side.

Anterior-Posterior (AP) Placement

AP placement is used during defibrillation for cardiac arrest patients. One pad is placed on the front over the sternum, and the other on the back near the spine.

AED Use for Patients with Pacemakers

Patients with pacemakers often have underlying heart disease and higher sudden cardiac arrest risk. However, AEDs can be used safely without interfering with the pacemaker.

AED Pads for Infants

Infant pads deliver lower energy shocks than adult versions. They are recommended for children under 55 lbs. If pediatric pads are not available, adult pads can be placed front and back.

AED Pad Placement for Children

Always follow the AED voice prompts and guidance from medical professionals.

In most cases, one pad goes on the front of the chest and one on the back, similar to infant placement.

Placement may vary based on age, size, and the AED model.

Children over 55 lbs may use standard adult pads.

Clothing Requirements

Pads must stick directly to skin. Expose the entire chest area for proper adhesion and function.

Removing Jewelry

All metal jewelry must be removed before using the AED. Metal can interfere with the shock and cause injury.

Step-by-Step AED Pad Application

Find the AED and open the case.
Turn on the device and follow audio and visual instructions.
Expose the patient’s chest.
Remove pad liners and apply one pad on the upper right chest and one on the lower left chest.
Attach the cables to the AED.
Wipe away sweat, hair, or dirt if needed.
Step back and let the AED analyze the heart rhythm.
If a shock is advised, press the shock button and stay clear.
After the shock, perform CPR as instructed.
Continue until emergency help arrives.
